Travel Request Form: Terms & Conditions

By submitting this form, I confirm that I have provided accurate spellings of names, accurate dates of birth, and other required personal information for myself and all other travelers included in this booking. I acknowledge that it is my responsibility to verify the accuracy of these passenger details when the final booking is completed.

I understand that any errors or omissions in the information I have provided may result in additional fees or penalties, and I accept full responsibility for these charges. I agree to notify Brittany Heyward, Fora Travel Advisor, of any corrections promptly. I also understand that any changes or corrections requested after the booking has been finalized may be subject to availability and additional costs, which I agree to cover.

Planning Fee: I acknowledge that there will be a planning fee associated with the creation of my personalized itinerary. Through email correspondence or via the complimentary intake call, the travel goals for my trip will be discussed, and a detailed understanding of any associated fees based on the unique aspects and complexities of my trip request will be provided.

Communication Protocol: I understand that the primary methods of communication during the planning process will be via email or scheduled phone calls. This approach is intended to provide the highest level of service and ensure no important information is overlooked. I agree to refrain from using text messages or social media for questions related to my trip, as this may compromise accuracy and clarity. Before my departure, I will be provided with the best local contact information for prompt assistance after business hours. I understand that unscheduled calls and texts should be reserved for in-travel emergencies.

Bundled Rates: I understand that some proposals may include bundled rates from suppliers without itemized breakdowns of individual components. This practice protects confidential, contracted rates from local vendors and ensures a seamless, holistic approach to my travel plans. I acknowledge and agree that my proposal may be presented as one packaged rate, and itemized pricing will not be available.

Payment and Cancellation Policies: I understand that a deposit may be required to secure my booking and that the final payment must be made by the specified deadline. I acknowledge that cancellations or changes to my booking may result in fees, as outlined in the cancellation policy provided with my proposal. I agree to these payment and cancellation terms.

Travel Insurance: I understand that travel insurance is strongly recommended to protect my investment against unforeseen circumstances, including trip cancellations, delays, and medical emergencies. If I decline to purchase travel insurance through Brittany Heyward, Fora Travel Advisor, or through my own insurance coverage I accept full responsibility for any costs incurred due to these circumstances.

Liability Waiver: I acknowledge that Brittany Heyward, Fora Travel Advisor, acts as an agent for the suppliers providing travel services, and as such, is not responsible for any changes, cancellations, or disruptions caused by these suppliers. I agree that Brittany Heyward, Fora Travel Advisor, is not liable for any losses or additional expenses due to delays, changes, or cancellations beyond her control.

Privacy and Data Protection: I understand that the personal information provided on this form will be used solely for the purpose of planning and booking my travel arrangements. I acknowledge that this information will be handled securely and in accordance with applicable privacy laws.

Client Responsibilities: I understand that it is my responsibility to ensure that I and all travelers in my party have the necessary travel documents, including passports, visas, and any required vaccinations. Brittany Heyward, Fora Travel Advisor, is not responsible for any issues or costs arising from failure to obtain these documents.
